Referral Guidelines
- Ages 2 to 17.5 years with a BMI ≥ 95th BMI percentile
- We offer health and wellness programs that vary based on availability and patient eligibility.
- We will inform families of what programs are available and will communicate with medical providers for ineligible patients.
Prioritized patients fall into the following categories:

Ages 2 to 17.5 years: Obesity with a major comorbidity, such as diabetes mellitus Type 2, hypertension requiring medications, moderate to severe obstructive sleep apnea, severe nonalcoholic steatohepatitis (NASH), pseudotumor cerebri, Blount's disease and slipped capital femoral epiphysis (SCFE), dyslipidemia, pre-diabetes (A1c 5.7-6.4) or provider concern for significant impairment in quality of life

Ages 12 to 17.5 years with severe obesity (≥ 99th BMI percentile) and interested in medications

Ages 15 to 17.5 years with a BMI ≥ 40.
