Each year, asthma is the cause of more than 3,500 asthma-related visits to our hospital, and it is our most common diagnosis. Now, we are working to improve quality of life for these children with Changing High-Risk Asthma in Memphis through Partnership (CHAMP), made possible with a $2.9 million Health Care Innovation Award from the Centers for Medicare and Medicaid Services (CMS).
In this program, physicians, nurses, respiratory therapists and community health educators support families to better manage asthma and address triggers that cause attacks. Other aims of CHAMP include:
- reduce deaths from pediatric asthma
- reduce Emergency Department and urgent care visits
- reduce avoidable hospitalizations
- reduce asthma exacerbation or episodes
We developed a health care coordination team for pediatric asthma patients. The team improves coordination of care between providers, teaches better asthma self-management and engages the community in caring for high-risk patients.
